Vitality Hospice & Palliative Care is seeking an exceptional Per Visit Registered Nurse to join our team. Schedule: Per Visit - Every Other Saturday & Sunday Responsibilities As a Hospice Visit Nurse, you will:
• Provide professional nursing care, including admissions, initial comprehensive assessments, and ongoing evaluations.

• Develop, implement, and update individualized plans of care based on patient needs.

• Collaborate closely with the hospice physician and full Interdisciplinary Team to ensure appropriate interventions.

• Oversee and support the work of home health aides, social workers, and chaplains.

• Educate patients and caregivers to promote safety, comfort, and understanding.

• Maintain high standards of documentation, compliance, and clinical excellence.

• Contribute to continuous improvement efforts to elevate the quality of care across the agency.

Qualifications • Active PA Registered Nurse (RN) license

• Minimum 1 year of RN experience

• Willingness to travel within assigned service areas

• Valid driver's license, reliable transportation, and active auto insurance

• Strong communication and interpersonal skills

Pay: $50.00 - $60.00 per hour
