Job Description
Schedule: Per diem visits - weekend rotations available - day time hours
Salary Range: $36.00 - $38.00 an hour

Responsibilities
• Deliver professional nursing care throughout the full nursing process within your scope of practice.
• Conduct comprehensive assessments of patients and families, contributing to personalized care plans.
• Complete various visit types as required to address patient and family needs.
• Administer medications and treatments according to physician orders.
• Identify nursing diagnoses based on health assessment data and respond appropriately.
• Document accurate and timely clinical notes, including pain and symptom management outcomes.
• Communicate any changes in patient condition to physicians and obtain necessary orders.
• Collaborate with community health partners for seamless care transitions.
• Ensure that equipment and necessary services are arranged and available for patients.
• Enhance clinical skills through ongoing education and in-service training.

Qualifications
• Active RN license in the state of employment.
• Graduate of an accredited Nursing program.
• Previous experience in hospice, home health, or a related care setting preferred.
• Flexibility and confidence in handling diverse patient assignments.
• Strong assessment skills and sound clinical judgment in high-pressure situations.
• Excellent documentation practices and communication abilities.
• Valid driver's license and willingness to travel locally.
