Registered Nurse Hospice AND Palliative Care

FULLY REMOTE
$60/HOUR
TEMPORARY ASSIGNMENT
We are seeking experienced Registered Nurses (RNs) or Nurse Practitioners (NPs) with a strong background in Hospice, Palliative Care, or Advanced Illness Management.
Ideal candidates will have experience providing Care Management or Care Coordination for patients with advanced or serious illnesses.
QUALIFICATIONS
• Active New York State RN or NP license - REQUIRED
• Minimum 3-5 years of experience in Care Management, Hospice, Palliative Care, Advanced Illness Management, or a related field
• Strong knowledge of Advanced Care Planning
• Experience with Goals of Care discussions
• Strong understanding of end-of-life care principles
• Care Management and Care Coordination experience
• Associate’s Degree in Nursing required
• Bachelor’s Degree preferred
• CHPN certification is a plus
Candidates with Advanced Illness Management, Palliative Care, or Hospice experience in a Care Management or Care Coordination capacity are strongly preferred.
PAY: $60/HOUR
100% FULLY REMOTE
