Registered Nurse, Hospice

LifeBridge Health, Inc. seeks a compassionate Registered Nurse for our Hospice team. This role provides holistic end-of-life care across home and facility settings, focusing on pain and symptom management, patient and family education, and emotional support. The RN conducts comprehensive assessments, develops and updates care plans, administers medications and treatments, and coordinates with physicians, social workers, chaplains, and therapists. Responsibilities
• Provide compassionate hospice nursing care in home and facility settings.
• Perform comprehensive patient assessments and ongoing reassessments.
• Develop, implement, and update individualized hospice care plans.
• Manage pain and complex symptoms through medications and interventions.
• Educate patients and families on disease process, comfort measures, and end-of-life expectations.
• Coordinate care with physicians, social workers, chaplains, therapists, and aides.
• Document assessments, interventions, and outcomes in the EMR accurately and timely.
• Participate in on-call rotation and respond to urgent patient needs.
• Support families through grief, coping, and use of community resources.
