Role Description The Registered Nurse Case Manager Hospice is a full-time, on-site role based in Temple, TX. This role is responsible for coordinating and delivering hospice nursing care to patients in accordance with individualized care plans, clinical guidelines, and regulatory standards. Day-to-day tasks include performing patient assessments, managing symptoms and pain, administering medications and treatments, and monitoring changes in condition. The RN Case Manager collaborates with physicians, social workers, counselors, and other interdisciplinary team members to develop and adjust care plans, ensuring continuity of care and appropriate utilization of services. Additional responsibilities include educating patients and families about disease progression and end-of-life care, supporting discharge planning, documenting clinical interventions accurately, and participating in team meetings and quality improvement activities.
Qualifications
• Strong clinical background with demonstrated skills in Nursing and Medicine relevant to hospice and palliative care.
• Experience in Case Management, including coordinating interdisciplinary care and advocating for patient-centered outcomes.
• Knowledge of Discharge Planning processes to support safe and appropriate transitions of care.
• Understanding of Utilization Management principles to ensure appropriate use of hospice services and resources.
• Current Registered Nurse (RN) license in the state of Texas, in good standing.
• Prior hospice, home health, or palliative care experience preferred.
• Excellent communication, documentation, and interpersonal skills, with the ability to support patients and families during end-of-life care.
• Ability to work collaboratively within an interdisciplinary team and manage a caseload effectively.
• Basic life support (BLS) certification and valid driver’s license if patient travel is required.
