Registered Nurse Case Manager Hospice 13

We are seeking a compassionate and dedicated Registered Nurse (RN) to join our hospice care team.
In this role, you will provide holistic, patient-centered care to individuals with terminal illnesses, ensuring comfort, dignity, and quality of life during their end-of-life journey.
As a Hospice Nurse, you will assume responsibility and accountability for assessing, planning, implementing, and evaluating individualized care plans.
You will utilize the nursing process, incorporating advanced technology, therapeutic interventions, evidence-based practices, and seamless coordination with an interdisciplinary team of healthcare professionals.
Your expertise and empathetic approach will support not only the patients but also their families, offering guidance and emotional support during this critical time.

Current RN License in state of practice.
Basic Life Support (BLS) Certification for healthcare providers.
Current driver's license, insured and reliable transportation and an acceptable driving record Ability to communicate effectively both verbally and in writing.
RNs with less than 12 months of working experience as an RN prior to joining Intermountain must obtain their BSN within 4 years of their hire date.
Bachelor's degree in nursing (BSN) from an accredited institution (degree will be verified).
Preferred Qualifications One year of nursing experience in Hospice, Home Health, Palliative Care, Oncology, Critical Care, or Medical-Surgical Nursing.
Experience providing care in patients' homes or community settings.
Strong communication, care coordination, and patient advocacy skills Bachelor of Science in Nursing (BSN) is required as of the job description's last update on 4/27/2025.

Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
