Job Description
JOB SUMMARY
- Provides patient care coordination, clinical support and skilled nursing assessment and evaluation for START patients.
- Performs triage of patient inquiries and coordinates appropriate resolution and follow-up.
DUTIES AND RESPONSIBILITIES
• Coordinates and collects all information necessary for new patient assessment and scheduling of new patient treatments.
• Conducts skilled nursing assessments that evaluate physical and emotional status of patients and communicates findings to physicians as appropriate.
• Prepares patient and equipment and assists physician with exams, treatment, and procedures.
• Provides health‑related education and counseling to patients to include self‑care, side‑effects management, and post‑treatment care.
• Triages patient inquiries and relates them to physicians.
• Plans strategies for solving patient‑care problems with other members of the patient‑care team.
• Coordinates patient care for treatment and ensures time‑appropriate follow‑up.
• Observes, evaluates and documents patient progress.
QUALIFICATIONS
• Current LVN license issued by the Texas State Board of Nurse Examiners or LVN licensure.
• At least 1-3 years of radiation/oncology experience or oncology certification preferred.
• Acute‑care nursing or coordination of care experience required.
