Role Overview



As QA Nurse / Compliance Coordinator, you will own clinical quality, regulatory compliance, and survey readiness for our home health program. You will conduct chart audits, drive performance-improvement initiatives, and ensure we are always survey-ready.



Key Responsibilities





- Conduct OASIS chart audits and clinical-record reviews to identify quality opportunities
- Lead performance-improvement projects targeting specific Star Ratings, HHCAHPS, and outcome metrics
- Prepare the agency for state and CMS surveys; manage corrective-action plans when needed
- Train clinical staff on documentation standards, infection control, and patient safety
- Track and report on quality indicators monthly to the leadership team
- Investigate complaints and adverse events; document findings and follow-up





Qualifications





- Current CA RN license + BSN preferred
- 3+ years home health nursing with at least 1 year in QA or QAPI
- OASIS-C certification (COS-C) strongly preferred
- Familiarity with CMS Conditions of Participation, Star Ratings, HHCAHPS
- Strong attention to detail and data-analysis skills
