As a Palliative Care Nurse Practitioner, you will provide essential patient and family care through palliative care consultations, focusing on specialty services for patients with serious illnesses and a prognosis of one year or less who are not yet ready or eligible for hospice care.

Schedule details: This position is PRN, requiring 2-3 days per week spread across the Union City and Huntingdon agencies.

Key Responsibilities:
• Utilize evidence-based palliative medicine practices efficiently and ethically to deliver comprehensive patient care, considering physical, emotional, spiritual, psychosocial, ethical, and educational criteria appropriate for the ages of patients served.
• Apply clinical expertise in palliative care to design and implement effective care plans.
• Gather detailed information from various sources, including medical records, family interviews, and caregiver insights, to perform thorough clinical assessments.
• Conduct a comprehensive evaluation using palliative care assessment domains, interpret findings, reconcile medications, generate problem-related diagnoses, and develop therapeutic intervention plans while assessing outcomes.
• Encouraged to achieve ACHPN certification after two years of employment or upon meeting experience requirements.

Required Qualifications:
• Current RN/APRN licensure in the state of practice.
• Nurse Practitioner/CNS with prescriptive authority.
• BCLS certification.
• 5+ years of clinical nursing experience.
• Valid driver's license and reliable transportation.

Preferred Qualifications:
• Advanced practice palliative care certification (ACHPN).
• Current CPR certification or the ability to obtain it within 90 days of hire.
• 1+ years of experience as an APRN in Hospice, Palliative Care, or Oncology.

The compensation for this specialist role typically ranges from $109,500 to $164,000. Total cash compensation includes base pay and bonuses and is influenced by factors such as local labor markets, education, and work experience, with potential increases over time based on productivity and performance. We comply with all applicable minimum wage laws.
