Dates of Coverage: Starting as soon as credentialed, for a 1-year contract.

Details/Requirements:
Start:
As soon as credentialed. Credentialing timeline is 2-3 months.
Seeking 4 days of coverage per week: 8:00am-5:00pm. Can potentially start with 2 days per week with a ramp up to 4 days by January 1st
Prefers full-time coverage if possible, but will patch with two providers doing 2 weeks per month each
Outpatient coverage
No weekends, No call
15 patients per day
50% Hematology 50% Oncology
EMR:
Epic
Support staff:
Other medical oncologist, care coordinators, RNs, MAs/LPNs, APPs, dietician, social work
Board Certified in Hematology and Oncology required
Active Ohio licensure or IMLC is required
