Position Summary
Stony Brook Community Medical, PC is seeking a compassionate, board-certified or board-eligible Palliative Care Physician to join our multidisciplinary team in Centereach, New York. The physician will provide comprehensive, patient-centered palliative care services to patients with serious, chronic, and life-limiting illnesses. This role focuses on improving quality of life by addressing physical symptoms, emotional well-being, psychosocial needs, and goals of care while working collaboratively with patients, families, caregivers, and healthcare providers.
Key Responsibilities
• Provide comprehensive palliative care consultations and ongoing management in outpatient, inpatient, skilled nursing, and/or home-based settings as assigned.
• Evaluate, diagnose, and manage pain and other complex symptoms associated with serious illness.
• Conduct goals-of-care discussions, advance care planning, and end-of-life care conversations with patients and families.
• Collaborate with primary care physicians, specialists, nurses, social workers, case managers, and other interdisciplinary team members to develop individualized care plans.
• Coordinate transitions of care across healthcare settings.
• Educate patients, families, and caregivers regarding disease progression, treatment options, and symptom management.
• Maintain accurate, timely, and compliant documentation within the electronic medical record (EMR).
• Participate in quality improvement initiatives and interdisciplinary team meetings.
• Provide consultation and education to healthcare staff regarding palliative care best practices.
• Ensure compliance with all federal, state, and organizational policies and regulations

Position Requirements
Qualifications
Required:
• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO).
• Board Certified or Board Eligible in Hospice and Palliative Medicine.
• Completion of an accredited residency program in Internal Medicine, Family Medicine, or another applicable specialty.
• Current New York State medical license or eligibility for licensure.
• DEA registration and New York State controlled substance certification.
• Excellent communication, interpersonal, and clinical decision-making skills.
• Commitment to compassionate, patient-centered care.
Preferred:
• Fellowship training in Hospice and Palliative Medicine.
• Experience in outpatient and community-based palliative care.
• Experience working with interdisciplinary healthcare teams.
• Familiarity with EPIC or other electronic medical record systems.
• Bilingual abilities are a plus.
Physical Requirements
• Ability to travel between practice locations as needed.
• Ability to perform patient examinations and clinical procedures.
• Ability to sit, stand, walk, and use standard medical equipment throughout the workday.
