Patient Population Adult Geriatric Approximately 1216 patients per day Clinical Responsibilities Provide medical oncology and hematology services Provide infusion-related oncology/hematology care Bone marrow biopsies Provider will work independently

Requirements Alaska license required at time of presentation Alaska DEA required BLS, ACLS, or PALS certification required Must be comfortable working independently Epic experience preferred Clean credentialing file required

Case Log Requirements Must provide documentation of Hematology/Medical Oncology services performed during the previous 24 months Case logs must demonstrate experience representative of the scope and complexity of requested privileges Case log requirement may be waived for providers who completed training within the previous year Additional Information Critical access setting Adult Level IV trauma designation Coverage approval currently noted through March 30, 2027"
