Licenses: Active Indiana License or IMLC
Board: Board Certified or Board Eligible
Certifications: BLS Required
Flu and COVID Vaccination Required or Approved Exemption
Case Logs Required

DUTIES & RESPONSIBILITIES
Provide Inpatient Hospice Coverage
Manage Hospice Admissions and Follow-Up Care
Participate in Night and Weekend Call Rotation
Complete Documentation and Order Entry in Epic
Coordinate Care with the Hospice Team
