Pride Health is hiring a Physical Therapist to support our client's healthcare facility in Madison, OH

Job Details:
Schedule: Monday-Friday | 8:00 AM-4:30 PM
Location: Madison, OH
Float / Coverage Area: Ashtabula, Lake, and surrounding counties
Job Type: Local contract
Contract Length: 13 Weeks
Pay Range: $50.00 to $55.00/hour*
Mileage Reimbursement Rate (if applicable): $0.49/ mile
*Pay offered is based on experience, expertise, credentialing, and education.

Duties: • Perform comprehensive physical therapy evaluations in the home setting.
• Develop and implement individualized treatment plans based on patient needs and goals.
• Provide skilled PT interventions to improve mobility, strength, balance, coordination, and functional independence.
• Monitor patient progress and modify treatment plans as clinically appropriate.
• Educate patients and caregivers on exercises, safety precautions, mobility techniques, and home programs.
• Coordinate patient care with physicians, nurses, therapists, and other members of the interdisciplinary team.
• Complete accurate and timely clinical documentation in accordance with home health and regulatory requirements.
• Manage an assigned caseload while meeting productivity expectations.
• Communicate changes in patient condition and progress to the appropriate care team.
• Maintain compliance with applicable clinical, safety, and documentation standards.
Requirements: • Education: Doctor of Physical Therapy (DPT) or equivalent accredited PT program
• License(s): Active Physical Therapist (PT) License - Ohio
• Certification(s): BLS/CPR preferred
• Years of experience: Home Health PT experience required
• Skills/Specialties/Technology: Home Health, PT Case Management, Patient Evaluations, Treatment Planning, Mobility & Functional Training, Care Coordination, Clinical Documentation, HCHB/Homecare Homebase preferred, EMR, OASIS familiarity preferred
