Physical Therapist - Home Health

The Physical Therapist - Home Health provides comprehensive therapeutic care to patients within their homes, focusing on post-surgical rehabilitation, geriatric care, chronic illness management, and neurological disorders. Working as part of a multidisciplinary team that includes nurses and aides, the therapist delivers tailored treatment plans while utilizing both paper documentation and electronic health records. This role requires local travel within a 10-mile radius to effectively support patient recovery and improve mobility in a home setting.



Responsibilities


- Conduct patient assessments to evaluate physical conditions and needs
- Develop and implement individualized treatment plans
- Provide exercise therapy and pain management interventions
- Improve patient mobility and functional abilities
- Educate patients and caregivers on therapy techniques and home safety
- Evaluate home environments for safety and equipment needs
- Document patient progress using paper and electronic health records
- Collaborate closely with nurses, aides, and other team members
- Recommend and facilitate use of assistive equipment
- Monitor patient progress and adjust care plans accordingly
- Plan and coordinate patient discharge from therapy services




Preferred Qualifications


- Minimum 1 year experience in physical therapy
- Doctor of Physical Therapy (DPT) degree
- Current state physical therapy license
- Skilled in therapeutic exercise and manual therapy techniques
- Experience with gait training and patient education
- Proficient in clinical documentation and home health care practices
- Strong communication and time management skills
