Job SummaryNeuLife Rehabilitation of Kentucky provides specialized residential rehabilitation services for individuals recovering from traumatic brain injuries, spinal cord injuries, and other neurological conditions. Our interdisciplinary team is committed to helping each person served achieve the highest level of independence, safety, and quality of life.We are seeking a compassionate and skilled Physical Therapist to join our team. In this role, you will provide individualized therapy services focused on improving mobility, strength, safety, and functional independence while collaborating closely with the interdisciplinary care team.ResponsibilitiesConduct physical therapy evaluations and develop individualized treatment plansProvide skilled therapy services based on functional goals and clinical needsMonitor progress and modify treatment interventions as appropriatePromote mobility, safety, and independence in daily activitiesCommunicate changes in condition, safety concerns, or barriers to progressCollaborate with physicians, nursing staff, case managers, and other interdisciplinary team membersComplete all documentation accurately and within required timeframesEnsure documentation supports medical necessity, compliance standards, and billing requirementsMaintain accurate records of all services provided and hours workedParticipate in care planning and interdisciplinary communication as neededPerform other duties as assignedQualificationsCurrent Kentucky Physical Therapist (PT) license requiredGraduate of an accredited Physical Therapy programExperience in rehabilitation, neurological rehabilitation, long-term care, or healthcare settings preferredStrong assessment, treatment planning, and clinical skillsExcellent communication and documentation abilitiesAbility to work effectively within an interdisciplinary team environmentPhysical RequirementsAbility to assist individuals served with mobility, transfers, positioning, and physical activitiesAbility to stand, walk, bend, reach, and remain active throughout the workdayAbility to safely perform duties in a rehabilitation environmentWhy Join NeuLife?Meaningful work helping individuals rebuild independence and improve quality of lifeCollaborative interdisciplinary team environmentCompetitive compensationComprehensive benefits package, including Medical, Dental, and Vision Insurance401(k) Retirement PlanPaid Time OffProfessional growth and development opportunitiesNeuLife Rehabilitation of Kentucky is an Equal Opportunity Employer
