Requirements


• MD or DO

• Board Eligible/Board Certified in Hematology/Oncology

• Current, unrestricted state and federal prescribing authority

• Active DEA

• Clean malpractice and background history

• Self-Query NPDB required and should be pulled within the month at presentation
