Role Description The Patient Navigator is a full-time hybrid role, based at Lifespan Cancer Institute at Rhode Island Hospital, with some work-from-home flexibility. In this position, the Patient Navigator guides individuals and families through the cancer care journey, helping them understand treatment options, appointments, and available support services. Daily responsibilities include coordinating referrals and scheduling, facilitating communication between patients and the care team, and assisting with access to resources such as financial counseling, transportation, and psychosocial support. The role involves conducting outreach, identifying and addressing barriers to care, and ensuring timely follow-up for diagnostic tests and treatments. The Patient Navigator documents patient interactions, maintains accurate records, and collaborates with clinical staff to promote a seamless, compassionate experience for every patient.
Qualifications
• Strong patient advocacy and care coordination skills, including the ability to guide individuals through complex healthcare systems.
• Excellent communication and interpersonal skills to interact effectively with patients, families, and multidisciplinary care teams.
• Organizational and time-management abilities to handle multiple cases, schedule appointments, and track follow-up activities.
• Comfort with basic technology, electronic health records, and remote communication tools to support hybrid work.
• Knowledge of oncology care or prior experience in cancer services, social work, nursing, or patient navigation is highly beneficial.
• Commitment to culturally sensitive, gender-neutral, and non-discriminatory practices when supporting diverse patient populations.
• Ability to work both on-site in Dallas, TX and remotely, maintaining confidentiality and professionalism in all settings.
• Associate’s or Bachelor’s degree in a health-related field, social work, or similar discipline preferred; relevant experience may be considered.
