NJ Palliative Care Solutions is hiring a part-time Nurse Practitioner or Physician Assistant to deliver hospital-based palliative care at Community Medical Center for patients living with serious illness. What you’ll do:
• Deliver inpatient palliative care at Community Medical Center — assessments, symptom management, goals-of-care conversations, and family meetings.
• Partner with the hospital’s care teams and the NJPCS interdisciplinary team.
• Complete timely documentation aligned with NJPCS clinical, billing, and quality standards.
What you bring:
• Active New Jersey Nurse Practitioner or Physician Assistant license and national board certification.
• DEA + CDS registration, or ability to obtain by start date.
• 2+ years of relevant clinical experience preferred (palliative, hospice, geriatrics, oncology, critical care, or complex chronic care).
• Palliative care certification (e.g., ACHPN / CHPN) preferred.
• Strong communication, reliability, clinical judgment, and comfort working in interdisciplinary teams.
