As a Hospice Nurse Practitioner, you will play a vital role in providing compassionate consultation in palliative care, symptom management, and supportive services tailored to the specific needs of our Hospice patients, as directed by their referring physicians.
Key Responsibilities:
• Conduct face-to-face visits with qualifying Hospice patients within specified time frames, sharing clinical findings with the Hospice Medical Director.
• Collaborate with the Hospice Medical Director and communicate recommended services to the primary physician, working alongside other healthcare providers as necessary.
• Initiate and coordinate comprehensive plans of care, documenting issues, goals, interventions, and patient/family responses to hospice care with the care team.
• Educate and guide patients and families on self-care techniques when appropriate.
• Maintain detailed and relevant clinical notes regarding patient conditions and any identified needs, reporting these insights to the Administrator and Hospice Medical Director.

Required Qualifications:
• Active unrestricted nursing license in your state of residence.
• Current certification as a Clinical Nurse Specialist or Nurse Practitioner.
• Certification by a recognized national organization such as ANCC, NBCHPN, or AANP.
• Current CPR certification.
• 5+ years of nursing experience.
• Valid driver's license and vehicle insurance, with access to reliable transportation or public transit.
Preferred Qualifications:
• Experience working collaboratively within an interdisciplinary team.
• Clinical experience in oncology, hospice, geriatrics, or intensive care settings.
• Palliative Care ARNP certification.
Compensation for this role typically ranges from $104,500 to $156,000. Total cash compensation includes base salary and bonuses, influenced by factors including local labor markets, education, and experience, with potential increases based on productivity and performance.
