Key Responsibilities:
• Conduct inpatient palliative care consultations for hospitalized patients with serious or life-limiting illnesses.
• Facilitate family meetings and engage in goals-of-care discussions.
• Collaborate closely with a team of professionals including nursing, social work, chaplaincy, and case management.
• Assist in ensuring smooth transitions of care and coordinate with hospice services when appropriate.
• Take part in quality improvement initiatives and contribute to program development.

Qualifications:
• Board Certified in Hospice & Palliative Medicine.
• Board Certification in Internal Medicine, Family Medicine, or completion of a Geriatrics Fellowship.
• Possess or be eligible for a Texas medical license.
• Exceptional communication skills and teamwork abilities.
• A strong commitment to compassionate, patient-focused care.
