We are seeking a dedicated and patient-focused physician who thrives in a collaborative medical environment. Responsibilities:
• Provide inpatient-only palliative care consultations for hospitalized patients dealing with serious or life-limiting illnesses.
• Lead and participate in family meetings and goals-of-care discussions.
• Collaborate with interdisciplinary team members including nursing, social work, chaplaincy, and case management.
• Support transitions of care, including coordination with hospice when appropriate.
• Participate in quality improvement initiatives and program development.
Qualifications:
• Board Certified in Hospice & Palliative Medicine.
• Board Certification in Internal Medicine, Family Medicine, or completion of a Geriatrics Fellowship.
• Possess a Texas medical license or the ability to obtain one.
• Strong communication and team collaboration skills.
• Commitment to compassionate, patient-focused care.
