Impact Health is seeking a compassionate and dedicated Palliative Care Nurse Practitioner to join our growing healthcare team in Lancaster, PA!!

Compensation and Benefits:

• Earning potential: $115,000–$200,000 (salary based on experience + easily achieved monthly bonuses)

• Flexible Schedule: Monday – Friday. No on-call, weekends, or holidays.

• Autonomy with Support to deliver independent care with access to physician-level consults.

Key Responsibilities

• Deliver person-centered care using evidence-based practices

• Collaborate with interdisciplinary facility teams

• Conduct assessments, develop treatment plans, and manage follow-up care

• Provide ongoing follow-up and chronic care management in Skilled Nursing Facilities (SNFs)

• Educate patients, families, and staff

• Complete timely documentation (within 24 hours)

• Compliance with facility, state and federal regulations/protocols.

Qualifications:

• Active license as a Nurse Practitioner (NP)

• Minimum 2 years of post-certification experience

• Experience in SNFs, hospitalist roles, emergency departments, or ICU preferred

• Proficient with EHRs and clinical technology

• Strong communicator with a collaborative mindset
