We’re looking for a Nurse Practitioner with a passion for palliative and/or hospice care to join our growing team. This is an opportunity to make a meaningful impact by supporting patients and families through complex health journeys with dignity and compassion.

What You’ll Do
• Provide comprehensive, patient-centered care to older adults in palliative or hospice settings
• Conduct assessments, diagnose conditions, and develop individualized care plans
• Collaborate with physicians and interdisciplinary teams to improve patient outcomes
• Support patients and families through serious illness, focusing on comfort and quality of life
• Prescribe and manage treatments in accordance with state guidelines and collaborative agreements
• Educate patients, caregivers, and staff on care plans and disease progression
• Coordinate care across settings and connect patients to appropriate resources

What We’re Looking For
• Active Missouri NP license (RN + NP)
• Certification as an Adult or Geriatric Nurse Practitioner
• Experience in palliative care, hospice, geriatrics, or complex care management preferred
• Palliative Care certification (or willingness to obtain)
• Strong clinical judgment and ability to work independently
• Passion for working with older adults and supporting families
