Job Description
Palliative Care Nurse Navigator - Emergency Department
This position will support the Emergency Department. The Palliative Care Nurse Navigator is a specialized registered nurse who guides patients and their families through serious or life-limiting illnesses. They coordinate care, triage referrals, facilitate goals-of-care discussions, provide education regarding disease processes, and help connect patients to community resources to improve their quality of life.
Minimum Education
Graduate of an approved school of nursing, B.S.N. preferred.
Minimum Work Experience
3 years of professional nursing experience.
2 years of palliative or hospice care preferred.
Required Licenses/Certifications
Current Vermont RN license.
BLS Certification through American Heart Associations.
Certified Hospice & Palliative Nurse (CHPN) or equivalent certification*
*Or obtained within 2 years of hire.
Required Skills, Knowledge, and Abilities
Demonstrates safe knowledge of nursing practice skills at all times based on theory and consistent with policy and procedure.
Demonstrates specialty knowledge and practice of nursing.
Excellent interpersonal, written, and verbal communication skills.
Ability to effectively communicate and relate information to individuals of varying ages from diverse socio-economic backgrounds.
In depth knowledge of utilization management criteria, complex medical and surgical patients, reimbursement and regulatory requirements including CMS guidelines.
Demonstrated strong attention to detail.
Demonstrated strong knowledge of basic computer skills.
Pay Range = $38.91 - $56.68
