Palliative Care Nurse Navigator

Rutland Regional Medical Center seeks a Palliative Care Nurse Navigator to guide patients and families through serious illness care. Responsibilities
• Serve as primary point of contact for patients and families navigating palliative care services
• Coordinate interdisciplinary care plans with physicians, nurses, social workers, and therapists
• Conduct comprehensive assessments of symptoms, psychosocial needs, and goals of care
• Provide education on diagnosis, treatment options, advance care planning, and community resources
• Facilitate family meetings and support shared decision-making aligned with patient values
• Monitor patient progress, adjust care plans, and manage referrals across care settings
• Ensure smooth transitions between inpatient, outpatient, and home-based services
• Document assessments, care plans, and outcomes in the electronic health record
• Collaborate in quality improvement initiatives and palliative care program development
• Participate in ongoing education, team meetings, and mentorship of staff on palliative principles
