Job Description

We are excited to welcome a compassionate Palliative Care Medical Practitioner to our dynamic team dedicated to enhancing the quality of life for members facing potentially life-limiting illnesses. This role primarily focuses on outpatient palliative care, providing thorough assessments, effective treatment plans, and thoughtful recommendations. Our practitioner will collaborate closely with an interdisciplinary team of skilled nurses, chaplains, social workers, clinical pharmacy specialists, and other healthcare professionals.

Outpatient visits will primarily occur in Kaiser Permanente medical offices and virtually, while inpatient responsibilities will be at Emory St. Joseph's and Emory Decatur Hospitals. As part of our team, you will work hand-in-hand with other physicians to address medical problems and illnesses, ensuring that patients receive care with a primary emphasis on their quality of life and functional ability. Key discussions may include advance directives, physical and psychosocial concerns, end-of-life care, and other appropriate topics.

We are looking for ideal candidates who are board certified in internal medicine, family medicine, or other relevant fields, and are also board certified in palliative care medicine.

Essential Functions:
• Conduct office, telephone, video, and inpatient visits as needed.
• Evaluate, manage, and recommend care for members referred to Palliative Care.
• Develop comprehensive medical care plans, addressing clinical issues, lab tests, medications, and referrals as necessary.
• Collaborate with other Palliative Care Team members, as well as the patient's primary care physician (PCP), Oncologist, Cardiologist, and other relevant specialists to optimize health and functional outcomes.
• Serve as a resource for clinicians and non-physician health professionals, enhancing care for patients with life-limiting illnesses.
• Engage in Palliative Care-related meetings to review team goals, activities, and quality metrics, providing recommendations for improved patient care.
• Participate in community presentations and educational activities, increasing awareness and knowledge of palliative care services among staff and the public.

Minimum Education and Experience Requirements:
• One year of post-residency experience caring for palliative care patients in outpatient and/or facility settings.
• Strong communication skills with patients, families, and a diverse healthcare team.
• Willingness to travel to various locations for patient care.

Preferred Education and Experience:
• Two years of post-residency experience delivering palliative care services in outpatient and/or hospital/facility settings.
• Experience working in an interdisciplinary team approach.
