Profiling / Transcribing: Reviews patient profiles to assure appropriate therapy based on patient specific information that includes at least the following: age, gender, diagnosis, co‑morbidities, relevant laboratory values, allergies, height, weight and BSA, pregnancy and lactation status. Reviews and transcribes all medication orders including specialized orders based on approved protocols accurately and timely verifying therapy, correct medication, route, dose and frequency. Makes recommendations for alternate treatments when necessary. Records relevant information and drug waste in EHR.
Dispensing Medications: Dispenses medications, prescriptions, miscellaneous items accurately and promptly. Processes requests via phone, window, fax, and computer in a professional and timely manner. Obtains medications when needed to provide optimal patient care using economical or alternative options and taking into consideration patient admission status (inpatient vs. outpatient). Legibly signs all paperwork and/or labels associated with these processes.
Checking: Checks unit dose accurately making corrections to patient profiles if necessary. Checks medications before being placed in automated dispensing machines and floor stock to assure accurate fill. Checks IVs, batching, patient own medications, repackaging and extemporaneous preparations for accuracy. Checks crash cart trays, rapid response boxes, and miscellaneous emergency kits. Legibly signs all paperwork and/or labels associated with these processes.
Sterile Preparations: Prepares, supervises and ensures aseptic compounding of IVs including antineoplastic agents correctly and in a timely manner promoting efficiency and cost effectiveness. Reviews levels of batched IV medications, batches, supplies to avoid waste, documents all required information, and assures batches are checked and put away promptly. Demonstrates a strong knowledge base of IV equipment, sterile technique, total parenteral nutrition, and safe handling of hazardous drug preparation and precautions.
Clinical: Participates in clinical initiatives. Initiates appropriate clinical interventions and clarifications with corresponding documentation. Demonstrates compliance with Pharmacy and Therapeutics Committee initiatives. Reviews pertinent information to monitor patients’ therapies and makes appropriate recommendations eliminating duplicate, unwarranted, or inappropriate therapy. Converts non-formulary medication requests to appropriate formulary alternatives whenever possible.
Drug Information: Provides drug information to physicians, nurses, other health care professionals, and patients/family members. Demonstrates and shares knowledge of various drug information resources and knows how to research and interpret data and information.
Education / Experience
Graduation from an accredited School of Pharmacy with PharmD required.
Qualifications
• Current active registration and licensure with the State of New Jersey Board of Pharmacy required.
• Previous hospital pharmacy experience (2+ years) preferred.
• Previous Oncology experience preferred.
