Oncology Nurse Practitioner

This Nurse Practitioner role involves comprehensive management of oncology patients in both hospital and office settings. The successful candidate will work collaboratively with physicians and colleagues to implement and coordinate care plans, ensuring timely access and continuity of care. This position supports the Patient-Centered Medical Home model by actively engaging with patients, families, and caregivers to promote understanding of care plans and self-management responsibilities.

Practice Info

• Hospital and/or office-based setting

• Focus on oncology patients

• Supports the Patient-Centered Medical Home (PCMH) model

• Covers outpatient clinics in Fulton County, Henry County, Holland-Sylvania and Maumee, as needed

Responsibilities

• Provide direction to staff regarding patient care

• See outpatients in the office at their scheduled appointment times and same-day appointments for urgent/emergent needs

• See Infusion Room patients as needed for evaluation of patient and/or nursing concerns and respond to urgent needs

• Complete documentation of visits in EMR within 72 hours and ensure all visits are billed appropriately

• Order tests and prescriptions based on patient need

• Cover inpatient hospital rounding as needed and as credentialed, including completing progress notes and dictating discharge summaries

• Coordinate care between inpatient and outpatient office settings, including scheduling post-discharge follow-up appointments

• Manage office prescription line and respond to patient refill requests, and complete telephone assessments for clinical problems

Shift & Schedule

• Consistently arrives at work on time and adheres to regular work schedule

• May share weekend and holiday call with oncologist/hematologist

Requirements

• Active and appropriate state Nurse Practitioner licensure in good standing

• Demonstrated understanding of Nurse Practitioner theory, practice and medical care, medical equipment and instruments, and OSHA regulations

• Excellent interpersonal and communication skills

• Ability to analyze and determine solutions in order to best serve the patients

• BLS or CPR certification
