Hours: 8:00am to 4:00pm/9:00am to 5:00pm
Summary:
The Oncology Nurse Navigator (ONN) is a professional Registered Nurse with oncology-specific clinical knowledge who offers individualized assistance to patients, families and caregivers. The ONN works to coordinate high quality, person-centered care through effective communication with the comprehensive oncology team throughout all phases of the cancer experience.
The ONN is a critical member of the care team identifying and eliminating barriers to receiving appropriate care and providing education and resources to facilitate informed decision-making. Navigation services include prevention and screening activities, facilitating care transitions, treatment, survivorship and end-of-life care.
The ONN acts as a liaison between patients, caregivers and the care team to optimize patient outcomes and facilitate efficient care transitions while serving as a resource to physicians and other healthcare professionals.
Responsibilities:
EDUCATION
• NYS RN license required
• Bachelor's degree in nursing from an accredited school of nursing required
• Certified Oncology Nurse preferred
• BLS certification required
Experience
• Five (5) years acute nursing experience, with minimum 3 years in oncology required
• Previous experience in case management or patient education preferred
KNOWLEDGE, SKILL AND ABILITY
• Excellent communication and listening skills, communication skills both orally and written for a variety of audiences
• Proficient technology skills to utilize the EMR, computer programs and technology to develop presentations, trend data and identify opportunities for improvement
• Highly organized, able to implement, coordinate and direct a variety of programs and services
• Ability to take initiative and provide self-directed leadership to ancillary team while driving results
• Able to work collaboratively with other healthcare professionals and providers to deliver patient-centered, interdisciplinary care

• Normal heat, light space, and safe working environment; typical of most office jobs
• Occasional exposure to one or more mildly unpleasant physical conditions
