• Pay: $100/hr
• Worksite Type: Outpatient Clinic (Free-Standing)
• Reason for Coverage: Staff Leveling
• Start Date: 9/11/2023 – Depends on Credentialing Time Frame & When Provider is leaving
• End Date: 1/12/24
• Work Schedule: M - F 8a - 4p For the Clinic – Evening Call NO
• Staff Information: Director, 1 APP, Nurse Manager
• Patient Information: 12 -14 PPD – Adult Population Follow-ups
• Required Procedures/Skills: 25% - Hematology & 75% - Medical Oncology – IP Consults
• Required Documents:
• NCCPA/AANP
DEA/BLS
NY License
