Oncology Clinical Social Worker, LCSW

Carle Health seeks an Oncology Counselor LCSW to provide compassionate psychosocial support to patients and families navigating cancer care. In this onsite role, you will perform comprehensive psychosocial assessments, deliver individual, family, and group counseling, and coordinate resources to address emotional, social, and practical needs. Working within a collaborative, research-informed oncology team, you will contribute to care planning, crisis intervention, and end-of-life support while maintaining thorough clinical documentation. Responsibilities
• Provide individual, family, and group counseling to oncology patients and caregivers.
• Conduct psychosocial assessments and develop patient-centered care plans.
• Coordinate resources, referrals, and support services across the care team.
• Collaborate with physicians, nurses, and allied health professionals to support integrated oncology care.
• Offer crisis intervention, grief, and end-of-life support.
• Advocate for patients' psychosocial needs and access to community resources.
• Maintain accurate, timely clinical documentation and comply with regulatory standards.
• Participate in case conferences, oncology team meetings, and quality improvement initiatives.
