Job Description
Join Mission Health as an Oncology Clinical Nurse Coordinator, where you'll be an integral part of our Healthcare & Medical Services team. This dynamic RN role is designed to enhance comprehensive oncology care, encompassing patient assessment, treatment planning, and navigation throughout the cancer care continuum. You will play a critical role in ensuring safe chemotherapy administration, timely follow-ups, and fostering clear communication among patients, families, and our interdisciplinary team. Key Responsibilities
• Facilitate the coordination of oncology patient care from diagnosis to survivorship or palliative care.
• Conduct thorough assessments and craft individualized care plans to meet patient needs.
• Collaborate with oncology providers to ensure safe chemotherapy and treatment workflows.
• Educate patients and families on their diagnosis, treatment options, management of side effects, and available resources.
• Act as the primary contact to streamline communication among patients, physicians, and the care team.
• Monitor patient progress, address clinical concerns, and manage symptom triage effectively.
• Ensure accurate and timely documentation in the EHR while maintaining compliance with regulations.
• Engage in quality improvement initiatives and participate in evidence-based practice projects.
• Collaborate with interdisciplinary team members to enhance care pathways and improve outcomes.
• Provide mentorship and support to newer staff to promote their professional development.
