As an After Hours Hospice RN, you will address patient needs during evenings, nights, weekends, and holidays, delivering peace of mind and compassionate care when it matters most.

Key Responsibilities as a Hospice RN - After Hours/On-Call:
• Report directly to the Administrator, Executive Director, or Patient Care Manager Senior.
• Deliver direct patient care and support during after-hours.
• Act as the primary connection between patients, families, physicians, and the hospice team during your shift.
• Respond quickly and compassionately to urgent calls, patient visits, and symptom management requirements.
• Assess patient and family needs to inform clinical decisions and implement care plans.
• Collaborate with an interdisciplinary team including LPNs/LVNs, CNAs, and additional support staff.
• Accurately document clinical visits and communications.
• Participate in quality assessment and performance improvement initiatives.
• Work independently while ensuring continuity and coordination of care with the daytime care team.

What We’re Looking For:
• Current RN license in the state of employment (or eligibility to obtain).
• Minimum of 1 year of nursing experience.
• A compassionate hospice heart: reliable, calm, and attentive in urgent care situations.
• Strong clinical judgment and the ability to support patients and families in emotionally charged settings.
• Comfortable providing care in various patient settings including homes and facilities.
• Valid driver's license, automobile insurance, and reliable transportation.
• Current CPR certification.

Preferred Qualifications (Not Required):
• 1+ years of experience in hospice, home health, oncology, or palliative care.
• Familiarity with symptom management, triage, and after-hours home-based care.
• Previous experience in on-call or overnight nursing roles.
• Strong documentation skills and proficiency in electronic medical records (EMR).
