Join Gentiva as an On Call Registered Nurse, where you will provide essential after-hours clinical support to home-based patients. You'll engage with hospice and home health patients to support their needs in the comfort of their homes.
Key Responsibilities
• Deliver after-hours triage support for home health and hospice patients via phone and in-person visits.
• Conduct thorough nursing assessments and respond to urgent changes in patient conditions.
• Administer medications, treatments, and symptom management according to care plans and established protocols.
• Collaborate with physicians and interdisciplinary team members to modify care plans as necessary.
• Ensure all assessments, interventions, and communications are accurately recorded in the EMR system in a timely manner.
• Provide education to patients and families regarding disease processes, medications, and home care instructions.
• Offer emotional support and assistance to patients and families during challenging end-of-life situations.
• Coordinate follow-up care and updates with the primary case management team to ensure continuity of care.
• Maintain compliance with clinical, regulatory, and safety standards while working independently in the field.
• Participate in the on-call rotation, ensuring consistent and reliable coverage.
Required Skills
