Registered Nurse - On Call RN - 7 Days On/7 Days Off

As an On Call Hospice RN, you will provide exceptional hospice care by responding to urgent patient and family needs after hours. You will play a vital role in ensuring continuity of care during evenings, overnight hours, weekends, and holidays—offering comfort, support, and peace of mind at critical times.

Work Schedule:
• 7 Days On/7 Days Off
• On-call coverage during evenings, overnight hours, weekends, and holidays for your scheduled week

This position is perfect for an experienced RN who values independence, meaningful connections with patients, and a unique work schedule that provides an entire week off between rotations while making a significant difference in the lives of patients and their families.

Your Responsibilities as a Hospice RN - After Hours/On-Call:

- Report directly to the Administrator, Executive Director, or Patient Care Manager Senior

- Provide direct patient care and support after regular business hours

- Serve as the primary communication link between patients, families, physicians, and the hospice team during your shift

- Respond promptly and compassionately to urgent calls, patient visits, and symptom management needs

- Evaluate the needs of patients and families to guide your clinical decisions and implement care plans

- Collaborate with an interdisciplinary team including LPNs/LVNs, CNAs, and other support staff

- Document clinical visits and communications accurately

- Engage in quality assessment and performance improvement initiatives

- Work independently while ensuring continuity of care by coordinating with the daytime care team

What We Seek:

Qualifications:

- Current RN license in the state of employment (or eligibility to obtain)

- A minimum of 1 year of nursing experience

- A compassionate disposition, remaining calm and dependable in urgent care situations

- Strong clinical judgment with the ability to provide support in emotionally charged environments

- Comfort providing care in patients' homes, facilities, or wherever they reside

- Valid driver’s license, automobile insurance, and reliable transportation

- Current CPR certification

Preferred Qualifications (Not Required):

- 1+ years of hospice, home health, oncology, or palliative care experience

- Familiarity with symptom management, triage, and after-hours home-based care

- Experience in on-call or overnight nursing roles

- Proficient in documentation and electronic medical records (EMR)
