Your Impact
As the On-Call RN Care Manager, you will:
• Deliver compassionate and direct patient care during after-hours shifts, including assessments, admissions, routine visits, and urgent calls.
• Conduct thorough evaluations of patients' physical, emotional, and spiritual needs.
• Collaborate with an interdisciplinary team to update and implement individualized care plans.
• Educate patients and their families about symptom management, medications, and the philosophy of hospice care.
• Maintain clear and accurate clinical documentation to ensure regulatory compliance.
• Lead and support field staff while adapting to urgent care needs.
• Participate in essential meetings and continuous education to uphold the highest standards of care.
About You
• Must have a current RN license and at least one year of experience in acute care, oncology, home health, or hospice settings.
• Possess strong critical thinking, communication, and organizational skills.
• Be proficient with electronic medical records (EMR) systems and comfortable with mobile technologies.
• Demonstrate sensitivity to end-of-life care needs and the emotional realities faced by families.
