Summary:

As an Oncology Nurse Coordinator, you will play a key role in guiding patients and their families through every step of their healthcare journey. Using the nursing process, you will assess, develop, implement, and evaluate individualized care plans while proactively anticipating patient needs. Serving as the primary clinical contact, you will provide support, education, and guidance to patients, families, and caregivers, ensuring they feel confident and informed.

In this role, you will coordinate multi-disciplinary and multi-site care, facilitate seamless communication among providers, and organize all necessary education and training for the care team. Utilizes the nursing process to assess, develop, implement, and evaluate individualized plans of care. Acts as the central point of contact for patient and caregiver inquiries related to care plans.

Proactively anticipates patient needs to ensure timely navigation. Assesses learning needs and provides education to promote adherence to established care plans. Enhances the patient experience by delivering exceptional customer service and promptly addressing concerns.

Performs all responsibilities aligned with the Nurse II role, with additional emphasis on care coordination, communication, and patient advocacy. Interacts with patients in person, via phone, virtual visits, and the patient portal to ensure care is well coordinated.
