Occupational Therapist Home Health | Huntersville, North Carolina

Deliver compassionate, patient-centered care as a Home Health Occupational Therapist, providing essential therapy services directly in client residences. This role is a 13-week contract opportunity structured to support both patient progress and professional fulfillment.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active Occupational Therapist (OT) license in good standing for the state of practice

• At least one year of recent home health OT experience (field experience preferred)

• Previous experience using Home Care Home Base (HCHB) strongly preferred

• Familiarity with OASIS assessment and documentation

• Reliable transportation, valid drivers license, and current auto insurance

• Ability to work independently and maintain an organized, field-based caseload

• Strong clinical assessment, communication, and time management skills

Primary Responsibilities:

• Perform comprehensive evaluations and OASIS assessments as needed for start of care, resumption, recertification, and discharge

• Develop individualized care plans addressing ADLs, upper extremity function, cognition, and home safety

• Deliver therapeutic interventions including self-care training, adaptive equipment instruction, energy conservation, and environmental modifications

• Recommend and assist clients in acquiring necessary durable medical equipment (DME)

• Update care plans and communicate patient progress to physicians and the interdisciplinary care team

• Educate patients and caregivers on optimizing independence, fall prevention, and use of adaptive strategies

• Ensure timely, accurate documentation within HCHB

• Actively participate in interdisciplinary meetings, case conferences, and ongoing professional development

• Adhere to all regulatory and agency standards, including Medicare guidelines
