Job Description
Lily Medical Group is a well-established, family-owned healthcare practice with more than ten years of experience serving residents throughout Wisconsin. We are expanding our compassionate and skilled clinical team and are seeking a motivated Nurse Practitioner (NP) or Physician Assistant (PA) to provide care to the senior adult population throughout Greater Milwaukee and the surrounding counties.
This position will have a strong focus on completing Medicare Annual Wellness Visits and Palliative Care Visits for patients residing in assisted living communities, independent living communities and senior care settings.
The ideal candidate will be comfortable performing comprehensive health assessments, identifying care needs, managing chronic conditions, discussing goals of care, and helping patients and families navigate complex medical decisions. Previous experience in palliative care, geriatrics, hospice, primary care, or senior care is highly preferred.

Primary Responsibilities
• Complete Medicare Annual Wellness Visits for eligible patients
• Perform comprehensive health and risk assessments
• Review medical histories, medications, preventive care needs, and chronic conditions
• Develop and update individualized prevention and care plans
• Conduct palliative care evaluations and follow-up visits
• Assess symptoms, functional status, quality of life, and patient care needs
• Discuss goals of care and treatment preferences with patients and families
• Coordinate care with primary care providers, facility staff, hospice agencies, specialists, and other members of the healthcare team
• Accurately and promptly document all visits in the electronic health record
• Identify changes in condition and make appropriate referrals or treatment recommendations
• Provide compassionate, patient-centered care to seniors with chronic, serious, or life-limiting illnesses
• Work independently while maintaining communication with the Lily Medical Group clinical and administrative teams
• Travel to assigned patient locations throughout Greater Milwaukee and surrounding counties
Qualifications
• Current Wisconsin Nurse Practitioner or Physician Assistant license
• Current national board certification, as applicable
• Experience providing care to geriatric or medically complex patients
• Experience with Annual Wellness Visits, preventive care, primary care, or chronic care management is preferred
• Palliative care or hospice experience is highly preferred
• Strong clinical assessment, documentation, and communication skills
• Ability to work independently and manage an assigned patient schedule
• Commitment to compassionate, high-quality, patient-centered senior care
• Reliable transportation and the ability to travel throughout Greater Milwaukee and surrounding counties
