Nurse Practitioner - Lung Cancer Screening - Remote - Days

This is a full time day shift position, supporting lung cancer screening shared decision making conversations with patients.

This advanced practice provider will play a critical role in the early detection of lung cancer through evidence-based screening, patient education, shared decision-making, tobacco cessation counseling, and care coordination.

The Nurse Practitioner will work collaboratively with primary care providers, pulmonologists, radiologists, thoracic surgeons, oncology specialists, and care management teams to ensure patients receive timely, high-quality, and patient-centered care throughout the screening and diagnostic continuum.

Key Responsibilities
• Conduct comprehensive lung cancer screening consultations and assessments.

• Perform and document CMS-compliant shared decision-making visits for eligible patients.

• Evaluate patient eligibility for low-dose CT (LDCT) screening based on current national guidelines.

• Order and interpret screening-related diagnostic studies within scope of practice.

• Communicate screening results and develop follow-up care plans.

• Manage patients with abnormal findings and coordinate referrals to specialty care.

• Provide smoking cessation counseling and connect patients with appropriate resources.

• Collaborate with primary care practices to improve screening rates and reduce disparities in care.

• Participate in quality improvement initiatives and program performance monitoring.

• Maintain accurate clinical documentation and ensure compliance with regulatory and accreditation standards.

• Support patient navigation activities to improve adherence to recommended follow-up.

• Participate in community outreach and educational initiatives to increase awareness of lung cancer screening.

Required Criteria
• Master of Science in Nursing

• NY Nurse Practitioner license (or willingness to obtain)

• FNP board certification through ANCC or AANP

Preferred
• Experience in pulmonology, oncology, primary care, preventive

• medicine, or population health.

• Experience working with multidisciplinary teams and quality improvement initiatives.
