ABOUT THE ROLE

As a vital member of Daymark’s team, the Nurse Practitioner will collaborate with our care team to deliver our innovative cancer care model to patients in their homes and through virtual platforms. Your mission is to provide empathetic and high-quality care while continuously enhancing our operational processes to promote effective patient engagement. This dynamic role requires a proactive attitude, as you will be involved in direct patient care and contribute to building a top-performing clinical team. You will work closely with our clinical operations team to implement evidence-based oncology care while addressing both clinical and non-clinical patient needs.

WHAT YOU'LL DO:
• Provide Clinical Care: Deliver direct patient care both in home and via virtual platforms, conducting assessments, prescribing medications, and documenting patient interactions appropriately.
• High Acuity Triage: Assist in navigating and providing care for high acuity patients while supporting a broader population, collaborating with local oncology practices through daily standups.
• Comprehensive Patient Support: Offer consistent clinical support across the full care continuum, from active treatment to survivorship or end-of-life care.
• Build Trusting Relationships: Foster strong relationships with patients and their support networks by emphasizing empathy and active listening in all communications.
• Innovative Care Model Application: Collaborate with clinical operations to refine and implement our care model, ensuring timely and appropriate interventions for cancer patients.

WHAT WE ARE LOOKING FOR:
• Experience: Must be an active Nurse Practitioner or Physician Assistant with at least 4 years of clinical practice, preferably in oncology, home care, palliative care, or hospice. Certifications like Oncology Nursing Society and experience with value-based or home care are a plus.
• Organizational & Technology Skills: Exceptional multitasking skills, comfortable in a fast-paced environment, and proficient with tools like Slack, Zoom, Google Suite, and EHR systems.
• Problem-Solving Skills: Ability to engage in clinical problem-solving and contribute effectively to patient care solutions.
• Action-Oriented Mindset: Ability to identify priorities and ensure that both urgent and important tasks are managed efficiently.
• Adaptability: Experience in launching new programs and comfort with changing processes to enhance patient care.
• Player/Coach Capability: Ability to lead the team while also providing direct patient care as necessary.
• In-Person Care Delivery: Availability to conduct home visits to patients in Los Angeles, CA.
• Certification: Must be certified as a 103 NP or 104 NP by the California Board of Registered Nursing.

Additional Information:

The expected compensation for this role is $140,000 to $170,000 annually, based on location and experience. This is a full-time position with a working schedule of Monday to Friday for 8 hours each day.

All patient-facing positions at Daymark Health require Basic Life Support (BLS) certification and compliance with our Employee Vaccination and Testing policy. Candidates for in-home visits must have a reliable vehicle and a valid driver’s license.
