CHRISTUS St. Vincent is seeking a compassionate, experienced Nurse Practitioner to join our
Center for Healthy Aging in Santa Fe, New Mexico. This is an outstanding opportunity to provide meaningful, relationship-centered care to older adults in a supportive outpatient setting focused on helping patients maintain their independence, function, and quality of life.

Opportunity Highlights


- Full-time outpatient practice
- Flexible schedule:

- Monday–Friday, 8-hour workdays, or Four 10-hour workdays per week



- Patient volume of approximately 16+ patients per day
- Limited call responsibilities: Primary Care call rotation by phone only with nurse triage support
- Collaborative, team-based care environment dedicated to the unique needs of older adults

Position Responsibilities

The Nurse Practitioner will provide comprehensive outpatient care for an aging patient population, including:


- Conducting annual wellness visits and preventive health screenings
- Evaluating, diagnosing, and treating acute and chronic conditions
- Managing common geriatric concerns, including:

- Dementia and cognitive disorders
- Osteoarthritis and chronic pain conditions
- Urinary incontinence
- Multiple chronic medical conditions



- Ordering and interpreting diagnostic tests and laboratory studies
- Developing and managing individualized treatment plans
- Collaborating closely with physicians, specialists, caregivers, and interdisciplinary team members
- Supporting patient goals while promoting independence, safety, and quality of life


Qualifications


- Nurse Practitioner certification required
- Minimum of two years of Nurse Practitioner experience
- Active New Mexico Nurse Practitioner license, or ability to obtain
- Passion for caring for older adults and delivering patient-centered care
