Job Description
Job Title: Dually Licensed Nurse Practitioner (FNP & PMHNP)
Location: Graham, NC (Office-based & Approved Telehealth)
Schedule: In-person 2 days and virtual(Remote) 3 days
Job Type: Full-Time
Role Overview
Provide integrated primary care and psychiatric services to adults (18+) diagnosed with Intellectual and Developmental Disabilities (IDD) and Serious Mental Illness (SMI). Operating within a low-volume, high-touch model (averaging 4-5 patients per day), you will perform medical/psychiatric evaluations, manage acute and chronic conditions, prescribe medications, and coordinate person-centered care across residential and community-based programs.
Key Responsibilities
• Conduct comprehensive medical, psychiatric, and risk assessments for adult patients with IDD and SMI.
• Diagnose and manage acute/chronic primary care and psychiatric conditions.
• Develop integrated treatment plans, order/interpret diagnostic tests, and manage complex medication regimens.
• Provide preventive healthcare education, health screenings, and psychiatric medication management.
• Assess safety risks (suicide, self-harm, withdrawal) and coordinate emergency or specialty referrals as needed.
• Collaborate with interdisciplinary teams, including physicians, therapists, residential staff, and care managers.
• Deliver services through in-person clinic visits, residential home visits, and approved telehealth.
Work Schedule
• Full-time schedule based on medical and psychiatric needs.
• In-person 2 days and virtual(Remote) 3 days
• Requires weekend and holiday on-call coverage.
Qualifications
• Active North Carolina NP license..
• Dual national board certification as both a Family Nurse Practitioner (FNP) and Psychiatric Mental Health Nurse Practitioner (PMHNP).
• Active DEA registration and NC Collaborative Practice Agreement.
• 6+ months of experience preferred; new graduates with dual certification are welcome to apply.
• Prior experience working with adult IDD and SMI populations is strongly preferred.
• Valid driver's license with an insurable record (community/residential visits may be assigned).
• Must pass background checks, health care registry, and employment credentialing screenings.
