Job Description
Riverside is seeking a Nurse Practitioner to provide comprehensive services in a 24/7 mobile medical clinic, working under the supervision of a collaborating physician. This role involves evaluating health history, diagnosing medical conditions, developing care plans, and prescribing treatments. The Nurse Practitioner will coordinate and collaborate with patients, families, and other healthcare practitioners to deliver expert care.

• Deliver clinical and operational responsibilities of a 24/7 mobile medical clinic
• Work collaboratively with Riverside team members and community partners such as ALF and SNF staff members, patient family members and home health and hospice team members
Responsibilities
• Complete history and physical examinations, evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documenting findings in the patient's medical record
• Manage health conditions by developing an individualized plan of care, prescribing medications and/or treatments, obtaining consultations or making referrals and coordinating care services
• Effectively deliver the clinical responsibilities and associated operational responsibilities of a 24/7 mobile medical clinic under the supervision of the Medical Director
• Maintain performance and productivity standards, operational efficiency, excellent customer service and best practice standards
• Demonstrate critical thinking, expert care and best practice standards in the clinical decision-making process
• Maintain compliance with the established standards of practice of medicine for Advanced Practice Nurse/Nurse Practitioner/Physician Assistant in the state of practice
• Document findings and actions appropriately in the patient's medical record
• Maintain patient confidentiality at all times
Requirements
• Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program
• Master's degree with a minimum of one (1) year Nurse Practitioner experience
• Current nursing licensure in State and CPR certification
• Accreditation from an approved certifying body for advanced practice nursing as required by State
• Excellent observation, verbal and written communication skills, problem solving skills, mathematical skills; nursing skills per competency checklist
• The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job
• Must have and maintain a valid driver's license, maintain automobile insurance coverage and have access to an automobile
