Nurse Navigator (RN) - Ambulatory - Cancer Clinic - Full-Time - Days

Role Overview: The Nurse Navigator (NN) is a licensed registered nurse with disease-specific knowledge who provides individualized support to patients, families, and caregivers to ensure timely access to quality care across the cancer continuum.

Responsibilities

• Act as a subject matter expert, promoting patient-centered care and improving care access and timeliness.
• Collaborate with program leaders to evaluate community needs and identify service gaps.
• Perform the nursing process—assessment, diagnosis, planning, implementation, and evaluation—to deliver evidence-based, individualized care.
• Coordinate multidisciplinary care, ensure accurate documentation, and foster inclusive, respectful clinical environments.
• Support performance improvement initiatives, participate in professional development, and serve as a preceptor or mentor.
• Stay updated on advances in cancer care, promote research, and adhere to organizational policies and standards.
• Contribute to data collection, resource management, and interprofessional collaboration.

Qualifications & Skills

• Associates, Diploma, or Bachelor’s Nursing degree (Bachelor’s preferred).
• RN licensure (single-state or multi-state compact) and BLS certification required.
• Minimum of 3 years of nursing experience, with 2 years in disease-specific or outpatient care preferred.
• Strong critical thinking, time management, and emergency response abilities.
• Ability to work effectively in a fast-paced environment with excellent interpersonal skills.
