Location: Sinai Hospital of Baltimore
Address: 2401 W Belvedere Avenue, Baltimore, MD 21215
Schedule: On Site | Monday-Friday | 8:00 AM - 4:30 PM

Position Summary
Sinai Hospital is seeking a compassionate and experienced Nurse Navigator - Radiation Oncology to provide community-based care navigation for high-utilizer oncology patients. This role partners closely with physicians and a multidisciplinary team—including nursing, social work, and community health workers—to develop and implement individualized care plans that ensure quality outcomes and seamless coordination of services.
The Nurse Navigator plays a critical role in guiding patients and families through complex healthcare systems while advocating for their needs and supporting program development initiatives.
Key Responsibilities
Care Planning & Coordination
• Develop and implement individualized, multidisciplinary plans of care in collaboration with physicians and healthcare team members.
• Conduct comprehensive nursing assessments to evaluate patient and family strengths, needs, and ability to manage the disease process.
Patient Navigation & Advocacy
• Assist patients in navigating healthcare and community support systems to ensure timely access to care and follow-up services.
• Provide in-person support (hospital, Emergency Department, physician offices, patient homes) and telephonic outreach.
• Advocate for patients, families, and caregivers to address barriers and improve access to services and programs.
Quality & Program Development
• Conduct data collection, assessment, and reporting to support quality improvement and program growth.
• Participate in multidisciplinary initiatives to develop programs and resources for high-risk patients.
• Support the continued development of the community patient navigation program.
• Perform additional duties as assigned.
Qualifications
Required:
• Active Maryland RN license
• AHA BLS Provider certification
• Oncology nursing experience
Preferred
• Radiation Oncology experience
