Job Description
Hartford Healthcare seeks a compassionate Nurse Navigator to guide patients and families through the healthcare journey. In this role, you'll coordinate care, provide education, and serve as a trusted advocate across our network of hospitals and clinics. Working closely with physicians and the interdisciplinary team, you'll streamline appointments, facilitate referrals, and help remove barriers to care. Responsibilities
• Coordinate patient care across the continuum, serving as primary contact and advocate
• Assess patient needs, develop individualized care plans, and monitor progress
• Educate patients and families on diagnoses, treatments, and community resources
• Collaborate with physicians, nurses, and interdisciplinary teams to optimize outcomes
• Facilitate referrals, scheduling, and follow-up to reduce barriers to care
• Track quality metrics, document accurately, and support performance improvement initiatives
• Use EMR systems to maintain timely, accurate clinical and navigation records
• Provide culturally competent, compassionate support to diverse patient populations
