Accelerated Home Health Care is hiring a Medical Social Worker (MSW) to provide in-home psychosocial support to patients and families.Key Responsibilities:
Conduct assessments of psychosocial needs.Provide counseling and emotional support.Connect patients and families to community resources.Document interventions and communicate with the care team.Follow care plans and regulatory requirements.Minimum Certification & Licensing Requirements:
MSW degree and state licensure.Completion of agency onboarding/credentialing.Up-to-date TB test and reliable transportation.Preferred Qualifications:
