Job Description
Seeking a strategic clinical leader, the remote Medical Director for Transitional Case Management will manage the evolution of transitional care programs, support frontline teams, and contribute to organizational priorities, ensuring high-quality care for members navigating high-risk transitions.
Key responsibilities
Drive the development and implementation of the transitional case management program, focusing on measurable outcomes and clinical protocols
Provide clinical support and education to the frontline care team, participating in case conferences and collaborating on complex care situations
Lead special projects and strategic initiatives, developing clinical protocols and representing the organization in external meetings with stakeholders
Required qualifications
Board certified in medical oncology with at least 3 years of clinical experience caring for oncology patients post-fellowship
Unrestricted medical license with a willingness to obtain additional state licenses as needed
Experience with transitional care management programs or reducing avoidable readmissions in oncology or complex care populations is preferred
Proficiency in using technology, including video conferencing and electronic health records
Strong organizational skills and the ability to manage multiple tasks in a fast-paced environment
